If you have already left Thailand
The overseas instructions list application and testimony forms, passport and relevant visa copies, Thai entry and exit stamps for foreign applicants, photographs and two original fingerprint sets taken at a Thai embassy or local police station. They also describe payment evidence, return delivery and power-of-attorney documents for representative submission.
Contact the intended fingerprint provider before attending: it may have its own appointments and identification requirements. A scan of a fingerprint card is not the same as the original hardcopy sets described in the official instructions. Keep a copy of the submitted file and delivery tracking for your own records.
