Q1080. What alternative district office documents can replace a lost name-change certificate for a Thai police application?

Answered by NYC Visa & Translation
An officially issued replacement or certified extract of the name-change registration may replace a lost certificate, subject to Royal Thai Police acceptance. Contact the district office or municipal registration office responsible for the original record and request documentary evidence showing both the previous and current names. If the surname changed through marriage or divorce, the relevant registration record may help establish the connection, but ask the Police Clearance Service Center which document it will accept. A current house-registration record or ID card may show only your present name and may not prove the change itself. Avoid relying on an informal statement or photocopy without checking its acceptability first.
